Ryan White Medical Case Management
Our RW Medical Case Managers assist people living with HIV/AIDS in accessing services including medical, social, mental health, drug/alcohol services, housing, transportation, and food resources. Case Managers serve as advocates between clients and their medical care team, helping clients achieve and maintain medication adherence. We are client-centered, meeting people where they are, and letting them lead the way. We strive to improve our clients’ quality of life and treat all clients with dignity and respect.
Our Case Managers act as a friendly, helpful resource to clients and community partners. For clients who are newly diagnosed with HIV and/or are not currently in care, our Case Manager’s can answer questions about living with HIV and help with disclosing of status, facilitate entrance into medical care, accompany clients to their first appointments, and beyond if needed.
