Ryan White Case Management
Case Managers assist people living with HIV/AIDS in accessing services including medical, social, mental health, drug/alcohol services, housing, transportation, and food. Case Managers serve as advocates between clients and their medical care team. Case Managers help clients achieve and maintain medication adherence. We are client-centered, meeting them where they are, and letting them lead the way. We strive to improve our clients’ quality of life and treat all clients with dignity and respect.
Our Case Managers act as a friendly, helpful resource to clients and community partners. For clients who are newly diagnosed with HIV and/or are not currently in care, our Case Manager’s can answer questions about living with HIV and help with disclosing of status, facilitate entrance into medical care, accompany clients to their first appointments, and beyond if needed. They can also make referrals to other supportive services as well. Give our Case Managers a call today if you need our assistance in any way. We are here to help.
